Disease Management Programs
CareTeam provides specialized disease management programs for clients with certain conditions or illnesses. The programs are designed to address the specific needs associated with those illnesses and to improve the client’s living standards while they are in their homes.
Goals of Disease Management are to optimize patients’ health and compliance, while avoiding costly or dangerous complications.
1. Parkinson’s Disease
- Gait supervision to prevent falls
- Light housekeeping to remove potentially dangerous obstacles
- Specialized meal preparation when swallowing is difficult
- Assistance with feeding during tremor episodes
- Assistance with daily exercise program to increase muscle strength, improve coordination and mobility, and reduce muscular rigidity
- Encouragement to participate in therapeutic program, including social and recreational events to help reduce depression.
2. Alzheimer’s, Dementia, and Stroke
- Meal preparation to maintain nutrition and prevent kitchen accidents
- Patient supervision to prevent wandering
- Personal grooming to assist with bathing, dressing, and oral hygiene
- Social stimulation to combat loneliness or depression
- Toileting reminders to eliminate potential accidents

3. Arthritis
- Assist with active range of motion exercises to prevent joint stiffness
- Apply ice or heat to relieve localized pain
- Assist with painful tasks such as driving, bathing, dressing, housekeeping
- Arrange household goods for easy access (cabinets and closets)
- Apply proper transferring techniques to prevent further joint damage
- Assist with meal preparation and diet modification to prevent weight gain which adds pressure to aching joints
4. Heart Disease
- Increase patient compliance with dietary and/or lifestyle changes
- Supervise prescribed exercise or physical therapy activities
- Assistance with cooking and housekeeping
- Ensure proper supervisions and act in the event of an emergency
5. Diabetes
- Assistance with meal preparation and diet modification
- Increase patient compliance with medication regimen by use of medication reminders

6. Cancer Assistance
Patients fighting cancer have individualized needs. Our goal is to meet those needs by providing excellent, attentive care to cancer patients, ensuring that they have the assistance and the support they need to win their fight with cancer.

- Assistance with daily activities at onset of fatigue to conserve patient energy
